Residential Community and Life Sharing/Family Living Services

What We Believe About Life Sharing and Family Living

  • Life Sharing/Family Living in the natural, therapeutic environment of a family enhances both the physical and behavioral health of an individual.
  • Human relationships fostered and supported by a family unit are the foundation for personal development and social change.
  • All of our Life Sharing/Family Living environments are licensed by the Department of Human Services.
  • All Life Sharing/Family Living providers are all trained in the necessary areas of CPR, First Aid, medication administration, and fire safety.
  • Life Sharing/Family Living is particularly beneficial for those individuals who can and wish to function in a community setting but desire more natural supports and inclusion than may be provided in a Community Living Arrangement.

How We Provide Life Sharing and Family Living Services

  • We use a person-centered approach and all of the person’s needs are addressed when selecting a family to partner with to support the individual.
  • Planning focuses on the needs, preferences, interests and passions of the individual being served.
  • Service planning always involves the individual and their significant others and individual choice concerning where they wish to live and with whom they wish to reside is paramount.

How We Provide CLA Services

  • Each person interested in a Community Living Arrangement, along with his/her family members and/or advocate, is directly involved in choosing the community and type of home in which they wish to reside.
  • Individuals are introduced to and spend time with both prospective roommates as well as the staff members responsible for their support, and they are encouraged to voice their opinions and concerns throughout the transition process and ongoing.
  • Individuals and their family members work extensively with the program team in order to develop a plan of care that supports both the physical, as well as the behavioral health needs of the individual. Authorizations for additional supports available through Supportive Concepts for Families, Inc. may include: behavioral health support; nursing services; intensive staff support.
  • Supportive Concepts for Families, Inc. makes arrangements for any and all services the individual may require on a daily basis. Family members may have as much or as little involvement in these services as they choose. As the provider, Supportive Concepts for Families, Inc. is responsible for all needs of the individual twenty-four (24) hours a day, seven days a week.
  • Supportive Concepts for Families, Inc. strives to ensure that each individual becomes an active member of the community in which they reside and Supportive Concepts for Families, Inc. functions as a liaison between the individual and the community resources they wish to explore. These may include but are not limited to employment, volunteer opportunities, day programming services and recreational and socialization activities.
  • We understand that people grow and change and may encounter both physical and behavioral health issues as a result of life changes. We take our commitment to each person seriously and will advocate for the person during difficult times.
