Supportive Concepts for Families, Inc., a leading provider of Intellectual and Developmental Disability (IDD) services in Eastern and Central Pennsylvania, is seeking qualified candidates to fill the position of Community Home Supervisor. The position will provide direction and oversight to our staff who provide support to our Intellectual and Developmental Disability (IDD) consumers in a home setting in the Berks County, PA area.
– The Community Home Supervisor will:
- Function with autonomy, diplomacy and professionalism within the guidelines established by this organization

- Function with the highest esteem in dealing with all employees, supervisors, and consumers of this organization
- Be an advocate for all of the policies and protocols of the organization
– Responsibilities of the Job and Essential Elements:
- Manage the day-to-day operations of the home to include but not limited to: responding to the needs of consumers, training employees, monitoring the daily plan for work, making recommendations for improvement, managing the household budget, weekly grocery shopping, maintenance and household repairs/purchases, ensuring a safe working environment for staff and consumers, compliance with state regulations, and implementing new policies and protocols
- Encourage consumer participation in meaningful activities, including ATP, Farm Program, and/or Transitional Work Program
- Read and respond to emails daily
- Participate in Awake Night inspections
- Participate with Positive Approach Resource Team (PART) calls
- Perform, supervise, and direct the work of staff to include the following:
- Medication
- Schedule and attend consumer medical appointments
- Monitor medications for refills
- Complete requirements for becoming Medication Administration Observer
- Reconcile cycle medications upon delivery prior to the start of each month
- Monthly medical review
- Count medications as per policy
- Ensure staff administering medications have completed and are current in medication administration training
- Report all medication errors immediately according to agency policy
- On Call Responsibility
- Be accessible twenty-four (24) hours per day, seven (7) days per week
- Make arrangements with another Site Supervisor for call coverage if unavailable for calls
- Complete on call log for time worked and submit to payroll weekly
- Training
- Complete required training and training hours
- Conduct and observe medication practicums for employees
- Monitor staff compliance with required training and training hours
- Conduct in home orientation for all new employees
- Conduct Competency Based Training for current employees
- Provide documentation for all training to the Training Department
- Complete understanding of 6400 regulations
- Maintain home according to 6400 regulations and agency policy
- Complete monthly 6400 Site inspection
- Maintain consumer files and records according to 6400 regulations and agency policy
- Maintain staff training according to 6400 regulations and agency policy

- Review daily shift to shift information
- Elevate any concerns to immediate supervisor and/or Health Services/Nursing
- Complete Program Oversight as per agency policy
- Maintain Health Information Recording System (HIRS) in accordance with agency policy
- Complete daily oversight in accordance with agency policy
- Prepare monthly paperwork to include but not limited to medication logs, fire drills and goals
- Monitor proper completion of all required paperwork to include but not limited to medication logs, goal documentation, charts, fire drills, quarterly reviews
- Timely submission of all required paperwork to the office
- Reconcile house petty cash daily and submit to the office as required
- Reconcile consumer funds daily
- House Maintenance

- Maintain consumer records according to 6400 regulations and agency policy
- Maintain current personal inventory of consumers’ belongings
- Communicate information and concerns to the Program Coordinator
- Staff meetings
- Facilitate/conduct staff meetings with approved agenda
- Forward sign in sheet and agenda to Training Department
- Incident Management
- Report incidents according to the Incident Management policy
- Respectfully interact with outside agencies (funding entities, case managers, counselors, day programs, employers, family members)
- Update/identify consumer outcomes as needed
- Supervise Residential Specialists
- Complete in home orientation and other home specific training
- Utilize Person Directed Framework/Non-Violent Communication during interactions with consumers and employees
- Mentor and provide ongoing feedback and indicate areas of improvement regarding job performance
- Partner with consumers and employees to identify issues and resolve problems
- Monitor CLA hours worked and overtime per the house budget
- Complete accurate and timely employee performance evaluations prior to the due date
- Report all employee concerns to the Program Coordinator and the Human Resource Department
- Vehicle maintenance
- Complete vehicle maintenance form – report vehicle maintenance needs/requests
- Complete mileage logs and inspection forms
- Maintain company vehicles
Previous experience in the field of Human Services is preferred, and ongoing in-depth training is provided. All candidates must be reliable and free from child abuse offenses.
